NYHA Class

Definition

Limitation

Example

I

Ordinary physical activity does not cause undue fatigue, dyspnea, or palpitations

None

Can complete any activity requiring 7 METs:

  • Carry > 11 kg (24 pounds) up stairs

  • Carry load weighing 23 kg (50 pounds)

  • Shovel snow

  • Spade soil

  • Ski

  • Play squash, handball, or basketball

  • Jog or walk ≥ 6.9 km/hour (≥ 4.3 mph)

II

Comfortable at rest; ordinary physical activity causes fatigue, dyspnea, palpitations, or angina

Mild

Can complete activity requiring 5 but 7 METs:

  • Vigorous sexual activity

  • Garden

  • Roller skate

  • Brisk walking up to 7 km/hour (4.4 mph) on level ground

  • Carry an 11 kg (24-pound) load up stairs

III

Comfortable at rest; less than ordinary physical activity causes fatigue, dyspnea, palpitations, or angina

IIIa: Without recent HF exacerbation or recent resting dyspnea

IIIb: With recent HF exacerbation or recent resting dyspnea

Moderate

Can complete activity requiring 2 and ≤ 5 METs:

  • Shower or dress while standing

  • Moderate household cleaning (eg, washing windows, sweeping, vacuuming, kitchen activities)

  • Play golf

  • Walk up to 5.4 km/hour (3.4 mph) on level ground

  • Climb stairs, slowly

IV

Symptomatic rest; any physical activity increases discomfort

Severe

Symptoms with all activities

Cannot complete any activity requiring 2 METs (all activities listed above)

HF = heart failure, MET = metabolic equivalent of task, a measure of how much energy is expended compared to remaining at rest; mph = mile per hour.
