Moderate glaucoma in the right eye shows thinning of the neuroretinal rim with an increased cup:disk ratio, vertical elongation of the cup (cupping), pitting or notching of the rim, wedge-shaped dark areas that reflect retinal nerve fiber layer damage, and bayoneting of the vessels at approximately 9:00 and 7:00.
